Your order is placed by E-Mail or Mail Order, and a detailed purchase order will be sent to you based on the information you give us. Fill
out the PetBio & Questionare and send to us. Your purchase order will be based on the Pet Bio & Questionare. If you agree with the
AshWork contract on the reverse side of purchase order, please sign & date the front where indicated. Return the signed
purchase order with check or money order enclosed (or charge
it with *PayPal) together with boxed ashes. We accept Visa, Mastercard and American Express through PayPal. A minimum deposit of 30% for each purchase order total
is required, balance due on completion of your custom memorial artwork, or shipped C.O.D by Federal Express (U.S. only). Please
allow one to four weeks to ship your finished PetStone or TeraStone artwork. International orders must be paid in
full before shipment.

PetStone panels are painted in one of our FleckStone colors. Our panels can allow for various enhancements to be ordered for custom artistic expression. You choose
the FleckStone color and enhancements for added contrast.

"HOW TO SEND ASHES"
Ashes must be sent U.S. registered mail to: "AshWork Design"
Post Office Box 28 Tempe, Arizona 85280. Postal insurance is optional
but recommended. Pet cremation centers usually give you the ashes in a wood or tin urn. If you would like to keep
the urn, remove the ashes and place them in a sealed plastic bag. Package them in a postal approved cardboard box with enough
packing material so contents do not shift. Please send by registered mail and include the signed purchase order, which will
designate AshWork Design as your agent for the ashes. Work can not proceed without this signed document by the owner of the
pets ashes. Note: Ashes can only be sent by U.S. postal registered mail.
For international orders you must check with your countries postal service
for any restrictions on sending pet cremains by registered mail. Private carriers will not deliver to a U.S. Post Office box
and cremains must be sent registered mail in care of AshWork Design.

PRIVACY POLICY
When you purchase any of our unique memorial artwork we retain only the information that you provide. Specifically,
your name, address, phone number and e-mail address. Subsequent to ordering from AshWork, we may send you e-mail alerting
you to other news concerning PetStone & TeraStone artwork. At any time, if you prefer not to be contacted, simply
reply to the e-mail you receive asking to be removed from our in-house e-mail list and we will remove your name immediately.
We offer you assurance that we will not share or sell your name or any of the information you have provided without your written
permission.
